What the mod does is adds a merchant or 'skooma dealer' to every one of the major cities in the game, the merchants also sell things like alcohol, sleeping tree sap, lockpicks, and moon sugar. The merchants will also buy your various junk. The dealers are available 24/7 so you don't need to wait to get your fix. The 'kingpin and drug dealer' thing you brought up was started on this site, Project Spenny. But I don't think it was ever finished, or even reached Beta. There's still an entire subsection dedicated to it in the Skyrim mods category. There was this purple Skooma Drug Effect from a mod and Haelga with a Khajiit Head. So I thought it would be funnier and more realistic if you people would look weird, when you have taken skooma.
